Mini Session Questionnaire

Thank you so much for scheduling a mini session with me! They are 10 minutes of crazy, fast and, fun, along with capturing beautiful photos!

Please fill out the following form so I can familiar myself with those attending and be ready with my camera and posing ideas:)

A $50 deposit is required to hold your appointment. Please submit payment via Venmo @birdonawire If you do not use Venmo, Please contact me for other options. Thank you!

Please arrive a few minutes early so we are ready to start at your scheduled time. Latecomers may be subject to forfeit their appointment. Deposits are non-refunable but are deducted from the session balance. You may pay the balance anytime via Venmo or with cash at the session.
